Terms of Service

Agreement between you and Sidcom AB for use of forms.ax.

Acceptable use

You may not use forms.ax to collect credentials (passwords, PINs, full card numbers), to impersonate a brand, or for phishing. Forms are screened at publish time; violating forms are unpublished and accounts may be suspended.

Signatures

The signature field captures a plain consent signature (drawn image + timestamp + IP). It is not a qualified electronic signature and makes no such claim.

Plans and billing

Paid plans are billed monthly via Stripe. Exceeding plan limits does not lose respondent data; overage prompts an upgrade.

Liability

Service provided "as is". Sidcom AB's liability is limited to fees paid in the preceding 12 months.
